The boys find that their father had another son who he only became aware of at age twelve. Further, they realize John made something of a place for himself in the kid's life for the next four or so years. Birthdays at baseball games, fishing trips, driving, poker, and pool lessons. The kid's had the opportunity to become kind of perfect - an eagle scout, honor student, and now attending college in a pre-med track.
Sam and Dean react differently to the idea of Adam. Sam is open to the idea - insisting the research allows for the possibility. Dean considers it a trap and needs convincing which occurs mostly through photo evidence. Sam readily takes Adam under his wing and wants to train him for hunting to protect him from the Winchester Curse. Dean is equally certain they should respect the set up for ignorance and normalcy that John must have felt protected the kid.
But what about Mary? She's not absent from this.
Kate in John's embrace? (Where is this supposed to have been taken? I doubt Windom, Minnesota has that kind of busy background?)

Is very like the photo on Mary's nightstand in the pilot... the same shot Sam has with him at Stanford.

Then there are Kate's dressertop with its ceramic pink roses as Dean investigates and ...

But then, the episode isn't done with Mary! Dean is determined to find what is after Adam so that it will be safe for him to continue in the civilian life John apparently wanted for the kid. He's stuck and ends up in a bar and is greeted by the bartender, who is wearing a shirt almost a twin to the one worn by1973's hunter Mary.

This character seems to be an amalgam of women Dean has positive feelings about and is predisposed to accept. Besides the obvious physical resemblance to Mary, her name is Lisa Barton (just a tick away from Lisa Braedon who Dean thought might actually be mother to his own child). She is a no nonsense older bartender whose missing husband apparently worked with John many years ago on a case. (Gee, I'd like to know what Ellen is doing these days.)
I'd be very curious to hear other ideas about this Lisa. I'm thinking of her like Sam's teacher from After School Special. She's there to encourage Dean and help him focus. She says, "I was scared they stopped looking. Now you're here," with gratitude when Dean acknowledges he's there to look into her husband's disappearance. At that point he's able to start making connections between John's old case and the current threat against Adam.
Bigger than that (and because I can't bear to waste an opportunity to speculate), I'd wonder if she isn't encouraging Dean to keep looking... for the truth behind the secrets and lies filling season four. Has anyone been able to look back and find foreshadowing that Adam was going to turn out to be a monster? The strongest I can find is in Dean's "It's a trap" instinct. He turned out correct, of course, but I'm not sure the story was headed there while Mary was a presence in the episode.

Yeah, wasn't it interesting how much Mary was present in that episode without her even once being referred to directly? The roses, the woman with blonde hair, the white shirt with the red rose embroidery... With as much as Dean's view of his father is maturing this season it makes me long to see how he would react to meeting up with the adult-version of his mother again. That last look was full of pity and sorrow in In the Beginning, but she's fallen quite far from the pedestal he had her on in WIAWSNB. *sniff* Our Dean, he's growing up. *g*
Also, by the time In the Beginning hit, I think I pretty much settled on Roses/flowers=secrets, in the vein of the subrosa concept Sadelyrate pointed out and the image of flowers opening up petal by petal to reveal what's inside.
So, Mary's presence has always been associated with flowers, and she had her secrets.
Dean shows up at Sam's motel room door and the first scene in which their together there's this incongruous well-lit red flower glowing outside a second story window in between them - and all of Sam's secrets.
This episode was chock-full of flowers associated with anything that had to do with the ghouls, particularly once ghoul!Adam showed upon the scene, or with the secret of John's other family.

For what it's worth, I was always satisfied that the 'off' photoshopping in "What Is and What Should Never Be" was an indicator and/or result of appearing in a dream world.

And yes, I addressed the issue of the car in my episode meta too, and put it down to one of several ghoul lies because it didn't make any sense.

I think the season has been filled with calm reactions to bizarre circumstances and that Adam was following Sam's lead. When Adam called John, it was because he had no one else and a missing mother. I'm thinking a couple of big brothers were welcome, but Adam would have had his own issues once the crisis was passed?

The background to the John/Katy pic looks a lot like Duluth, MN, which is in the north of the state (Windom is in the flat southern part of MN), built on cliffs that go down to the edge of Lake Superior. The period of the houses in the background matches Duluth's look, and Duluth is built on the side of a hill, which is what that background looks like to me. Which is kinda interesting, because Duluth is: 1) a college town (looking for a place to send Adam?), and 2) a popular vacation area, at least 4 hours' drive from Windom.

I had a theory about the roses motif in 4x19, that the only character shown interacting with them is Dean, to show that he's morphed into Mary as much as Sam has morphed into John.
Oh yeah, why the HELL did he take off his wedding ring man? Grr.
